Homecare Nurse

Roles & Responsibilities

• Administer medications and monitor patient responses, ensuring adherence to prescribed treatment plans and recognizing potential side effects or complications.

• Provide wound care and management, including dressing changes and monitoring for signs of infection to promote healing and patient comfort.

• Collaborate with physicians and interdisciplinary teams to develop and adjust individualized care plans based on patient needs and progress.

• Educate patients and families on disease management, medication adherence, and lifestyle modifications to empower them in their health journeys.

• Support activities of daily living (ADLs) such as bathing, dressing, and meal preparation, ensuring patients maintain their dignity and independence.

Desired Candidate Profile

• Registered Nurse (RN) with a valid nursing license, ensuring compliance with state regulations and standards of practice.

• Minimum of 2 years of experience in homecare or community health settings, showcasing adaptability and hands-on patient care skills.

• Certification in Home Health Nursing (CHHN) or similar credentials, demonstrating specialized knowledge and commitment to homecare excellence.

• Proficiency in electronic health record (EHR) systems, enabling efficient documentation and communication within the healthcare team.
